The Center for Medical Cannabis processes applications for medical cannabis cards for Utahns with approved medical conditions.
Steps to get started
Below are the steps and timelines to receive this service:
1. Review important information about medical cannabis
Time to complete: 5 minutes
2. Create UtahID* account
*If you already have a UtahID account, you do not need to create another account.
Time to complete: 5 minutes
3. Log in to the Electronic Verification System (EVS) using UtahID to complete registration and application
Time to complete: 10 minutes
4. Locate and meet with a qualified medical provider who is registered to recommend medical cannabis
Time to complete: Varies based on scheduling
5. You will receive an email from [email protected] once your medical provider approves your certification. Follow the instructions to pay the medical cannabis card application fee on EVS
Time to complete: 15 days or less. Timelines may vary for patients under 21 or applying with special circumstances
6. Download medical cannabis card from EVS. Print or save to your device
Time to complete: Instantly available after approval
7. Meet with a medical cannabis pharmacist on your first visit to a medical cannabis pharmacy
Time to complete: Based on availability

1. Review how to become a qualified medical provider (QMP)
Time to complete: 5 minutes
2. Complete 4 hours of DHHS-approved continuing education
Time to complete: 4 hours
3. Create UtahID* account
*If you already have a Utah ID account to access the Controlled Substance Database
To find your Utah ID username, navigate to id.utah.gov and click on "My Account" in the top left-hand corner of the screen after logging in.
4. Log in to EVS using UtahID to complete registration and application
Time to complete: 10 minutes
5. Pay medical cannabis registration card application fee online
Time to complete: 5 minutes
6. DHHS reviews medical cannabis registration card applications to confirm if all requirements have been met.
Time to complete: 15 days or less
7. You will receive an email from [email protected] with instructions on how to access your qualified medical provider registration card
