All medical cannabis products must be purchased from a licensed medical cannabis pharmacy.
The Utah Department of Health and Human Services has issued 15 medical cannabis pharmacy licenses. 14 pharmacies are operational with the 15th pharmacy scheduled to open later this year. Please click on the Pharmacy Location tab to see a list of all licensed pharmacies.
The Utah Medical Cannabis Act requires that each licensed medical cannabis pharmacy have a Pharmacy Medical Provider (PMP) on-site and available to consult with cardholders during business hours. It is the responsibility of the PMP to review medical cannabis transactions and verify that all transactions are completed correctly. Each PMP must register with the Utah Department of Health. All employees of a medical cannabis pharmacy, with the exception of pharmacists and physicians, must be registered with the Utah Department of Health and Human Services as pharmacy agents. Please click on the Pharmacy Agent tab for more information on how to register as a pharmacy agent.
Medical cannabis pharmacies may sell products in their store or through home delivery. Home deliveries are performed by the pharmacy or by a courier company.
A license from the Utah Department of Health and Human Services is required to perform deliveries of medical cannabis. Each employee of the licensed courier is required to register with the Utah Department of Health as a courier agent. Pharmacy medical provider applicants must:
- Complete four hours of department-approved continuing education and submit certification of completion with registration application. Find out about approved CE courses by visiting our Continuing Education page.

A medical cannabis pharmacy agent is any employee of a licensed medical cannabis pharmacy (with the exception of pharmacists and physicians).
Prior to beginning work, medical cannabis pharmacy agents must be registered by the Utah Department of Health and Human Services through the Electronic Verification System (EVS).
Please note that applicants must have been offered employment at a licensed medical cannabis pharmacy in order to be issued a medical cannabis pharmacy agent card. Do not apply until you have been offered employment. Any fees paid will not be refunded if an applicant applies prior to being hired by a licensed medical cannabis pharmacy.
