Pharmacy medical provider (PMP)
Pharmacists licensed under opens in a new tabTitle 58, Chapter 17b, Pharmacy Practice Act and physicians who have the authority to write a prescription and are licensed under opens in a new tabTitle 58, Chapter 67, Utah Medical Practice Act, or opens in a new tabTitle 58, Chapter 68, Utah Osteopathic Medical Practice Act may become "pharmacy medical providers" (PMP). PMPs must register through the Electronic Verification System (EVS). Please note that applicants must have been offered employment at a medical cannabis pharmacy in order to be issued a pharmacy medical provider card. Do not apply until you have been offered employment. Any fees paid will not be refunded if an applicant applies prior to being hired by a licensed medical cannabis pharmacy.
PMP applicants must:
- Complete four hours of DHHS-approved continuing education and submit certification of completion with registration application. Find out about approved CE courses by visiting our Continuing Education page.
