Release forms
Patients can use this form to release their medical cannabis card information to another person or organization, such as an employer. It will send them how long you’ve had a medical cannabis card. You can choose if you’d like to release your cannabis purchase history to them, too.
Medical providers who do not have access to the electronic verification system (EVS) can use this form to request their patients' medical cannabis information from DHHS. It will send the provider information, such as how long the patient has had a medical cannabis card and their cannabis purchase history.
