    What is a medical cannabis certification?

    A medical cannabis certification is an electronic or paper record that verifies a patient meets the qualifications to use medical cannabis in Utah. These can only be submitted by the patient’s medical provider, called a recommending medical provider (RMP).

    You can submit a certification in the electronic verification system (EVS) or directly to a medical cannabis pharmacy through encrypted email, secure fax, or in-person delivery by the patient. 

    You must also submit a Compassionate Use Board (CUB) petition for patients who are younger than 21 years or who don’t have a qualifying medical condition. 

    How to submit a certification directly to a medical cannabis pharmacy

  1. Meet with the patient face-to-face for their first visit with you.

    You can meet with patients virtually or through telehealth for follow-up and renewal appointments.

  2. Complete a full medical assessment to assess if medical cannabis is a good treatment option.

  3. Complete the medical provider recommendation form for medical cannabis.

    Download the form, complete it, and sign it.
    Estimated timeline: 10 minutes

  4. Submit the form to a medical cannabis pharmacy.

    Send the form to a medical cannabis pharmacy through encrypted email, secure fax, or delivered by the patient.

  5. The pharmacy will submit your medical cannabis certification into the electronic verification system (EVS).

    The pharmacy will contact you to verify the information.
