How to renew your recommending medical provider (RMP) proxy registration in the electronic verification system (EVS)
Your recommending medical provider (RMP) proxy registration expires every 2 years. You will get an email from the Utah Department of Agriculture and Food (UDAF) 60 days and 7 days before it expires.
You will renew your registration in the electronic verification system (EVS). This process is similar to applying for an RMP proxy registration. You will use your same UtahID and EVS account.
