Complete REDCap petition information.
After a certification is submitted, you'll receive an email from [email protected] with a REDCap petition.
Look at the pictures under the "visual help" heading to see how to complete a REDCap petition.
Make sure you have completed the survey submission.
Surveys that are incomplete will not be reviewed.
You'll receive a confirmation after the survey was submitted via REDCap.
The Department of Health and Human Services will review the petition and follow up.
You may receive an email with questions or requests for further information. Once we receive all the necessary documentation, you will receive an email from [email protected] within 90 days regarding the approval or denial of the petition.
What is REDCap?
REDCap is a secure web application for conducting online surveys and collecting data. REDCap is a secure way for providers to submit patient petitions to the Compassionate Use Board.
When is REDCap used?
If a recommending medical provider does not have access to the electronic verification system (EVS), or if there is an error when entering the petition. Most Compassionate Use Board petitions are completed in the EVS.
How to submit a petition in REDCap
You'll receive an email from [email protected] with a link to the CUB petition through a REDCap survey after a certification is submitted. Complete the required fields to submit your petition. Incomplete petitions will not be reviewed.
