What is a medical cannabis paper certification?
A medical cannabis paper certification is a record that verifies a patient meets the qualifications to use medical cannabis in Utah. These can only be submitted by the patient’s medical provider, called a recommending medical provider (RMP). RMPs can either submit certifications directly in the electronic verification system (EVS) or by sending a paper certification form to a medical cannabis pharmacy.
Paper certification forms can be sent to a medical cannabis pharmacy through encrypted email, secure fax, or in-person delivery by the patient. The pharmacy medical provider (PMP) will enter the paper certification into the EVS.
