Who is a medical cannabis patient?

Medical cannabis patients are adults 18 years or older with a medical cannabis card.

How to apply

You’ll apply for a medical cannabis card in the electronic verification system (EVS). The EVS is an online database that holds all of the medical cannabis cards, recommendations, and purchase data for patients and medical providers in Utah. You must create an EVS account to apply for a medical cannabis card.

Go to the EVS

Click the "Log in" button on the EVS.

You'll sign in to UtahID before creating an EVS account.

Submit your medical cannabis application in the EVS.

Click "+ add application"

Select "Utah resident" from the drop-down and "I am applying for myself". Then click "Next".

Enter your information. Required information is marked with a red star. Then click "Next".

Click the “Add caregiver button” if you’d like to have someone pick up your medical cannabis if you can’t go yourself. Enter their information and click the “Save” button.

Click the “Yes” if you’d like to have your medical cannabis delivered somewhere other than your house. Enter the information and click the “Save” button. You can add more than one delivery location.

Read the patient acknowledgment statement and click the “Acknowledge“ box.

Read the medical research statement. If you want to opt out of medical research, click the “Opt out of medical research” box. Then click the “Submit for certification” button.

Meet in-person with a medical provider.

Your provider will complete a medical assessment to figure out if medical cannabis is a good treatment option for you. If it is, they’ll submit a certification in the EVS that verifies you meet the qualifications to get a medical cannabis card. Find a medical provider near you.

Pay the medical cannabis card fee.

You’ll get an email from [email protected] with instructions to pay your medical cannabis card fee after your provider submits your medical cannabis certification in the EVS.

Download your medical cannabis card from the EVS. You can print or save your card to your device.

You must show your card and a valid photo ID each time you go to a medical cannabis pharmacy or get a medical cannabis delivery. Find a pharmacy or home delivery service.

Meet with a medical cannabis pharmacist.

You will meet with a medical cannabis pharmacist before your first medical cannabis purchase. This can be in-person at a medical cannabis pharmacy or through a video or phone call if the pharmacy has that option. Call the pharmacy to see if you need to schedule an appointment to meet with the pharmacist.

The pharmacist will talk with you about your medical condition, symptoms, and health history to provide recommendations for products and amounts to try. They will also answer any questions you have.

Buy medical cannabis

You can buy medical cannabis after you meet with the pharmacist. After your first purchase, you can buy from any Utah medical cannabis pharmacy or get home delivery.

Your medical cannabis card will last for 1 year.

Your provider may choose a shorter expiration date if they feel that will be best for your medical condition. You can see your card’s expiration date in the EVS.
