Pharmacy Medical Providers

Pharmacists licensed under Title 58, Chapter 17b, Pharmacy Practice Act and physicians who have the authority to write a prescription and are licensed under Title 58, Chapter 67, Utah Medical Practice Act, or Title 58, Chapter 68, Utah Osteopathic Medical Practice Act may become “pharmacy medical providers.” These pharmacy medical providers (PMPs) must register through the Electronic Verification System (EVS).

Please note that applicants must have been offered employment at a medical cannabis pharmacy in order to be issued a pharmacy medical provider card.  Do not apply until you have been offered employment.  Any fees paid will not be refunded if an applicant applies prior to being hired by a licensed medical cannabis pharmacy.
